To obtain an application, call the Senior HelpLine at 1-800-252-8966, 1-888-206-1327 (TTY), or file on the Internet at Illinois Cares Rx or Illinois Benefits.
If you need help to file on the Internet or you do not have a computer with Internet access, call your local Area Agency on Aging or local senior center.
To qualify for Illinois Cares Rx, people with Medicare MUST:
- apply for Medicare’s "Extra Help" from Social Security and,
- be in a coordinating Medicare drug plan to maximize benefits.
You may file an application on the Internet at Social Security Online or you may obtain a form by calling:

How does it work if I have Medicare?
If you are enrolled in a coordinating Medicare drug plan, Illinois Cares Rx will pay monthly premiums for the coordinating Medicare drug plan and help pay other drug costs. You must follow the Medicare drug plan formulary.
In 2008 you will pay:
- $2.25 for generic covered drugs
- $5.60 for preferred brand name covered drugs
- $15 for specialty and non-preferred brand name covered drugs
- 20% plus co-pays after $2,510 in drug benefits have been paid on your behalf
NEW! Additionally, if you have HIV/AIDS, co-payments will be $2.25 for generic drugs and $5.60 for brand name drugs during the plan year for drugs on the ADAP formulary that are also on the Medicare Part D prescription drug plan formulary.
NOTE: Some drugs may require prior approval from your Medicare drug plan.
You will receive an Illinois Cares Rx card and Medicare drug plan card. Take both cards to the pharmacy.
How does it work if I don’t have Medicare?
Once you are determined eligible, Illinois Cares Rx will help pay your drug costs.
In 2008 you will pay:
- $2.25 for generic covered drugs
- $5.60 for preferred brand name covered drugs
- 20% plus co-pays after $1,750 in drug benefits have been paid on your behalf
NOTE: Some drugs may require prior approval.
You will receive an Illinois Cares Rx card to use at the pharmacy.
The Medicare drug plan open enrollment period is November 15 – December 31 each year.
If you are… a senior age 65 or older, or a person with disabilities:
- Single with an annual income less than $24,808 or
- Married with an annual income less than $32,916
You qualify for… Illinois Cares Rx BASIC
There is no citizenship requirement, but, if eligible for Medicare, you must apply for “extra help” from Social Security.
BASIC covers…
Prescription drugs for the treatment of Alzheimer’s disease, arthritis, cancer, diabetes, glaucoma, heart and blood pressure problems, HIV/AIDS (if eligible for Medicare), lung disease and smoking-related illnesses, multiple sclerosis, osteoporosis and Parkinson’s disease.
If you are… a senior age 65 or older:
- Single with an annual income of no more than $23,225; or
- Married with an annual income of no more than $31,264; and
- a citizen or qualified non-citizen
You qualify for… Illinois Cares Rx PLUS
If eligible for Medicare, you must apply for "extra help" from Social Security.
PLUS covers…
All classes of medically necessary prescription drugs.
To get Illinois Cares Rx drug coverage, you must apply on a Form IL-1363 each year.
